The information on this application is correct to the best of my knowledge. By signing this agreement I am committing to partnering with Countryside Kids Ministries in the spiritual development of children. I am committed to my personal spiritual growth, attending meetings and training events, attending corporate worship services at Countryside on a regular basis and supporting the purpose, mission, and vision of Countryside Covenant Church.

Your application will be submitted to Amy Brunk, Director of Children's Ministries. Once reviewed, you will be contacted about scheduling an interview. We do complete background checks on all adults who work directly with children at Countryside. At the time of the interview, you will need to provide your social security number to complete the background check.
